ABO Reverse Typing: Benefit or Burden?
The ABO blood group system stands out as the only system in which individuals naturally possess antibodies against ABO antigens they do not express. This unique feature is the primary reason for transfusion reactions when ABO-incompatible blood is administered. These naturally occurring antibodies are also crucial in confirming ABO blood types, especially through the reverse grouping method.
In this webinar, Peter C. Ligthart delves into common ABO discrepancies that arise between antigen (forward) typing and reverse typing, with a particular focus on missing or unexpected reactions in reverse typing.
He also critically examines the role of reverse typing in patient testing. Is it an essential component of ABO determination, or is it performed simply because it's readily available?
Finally, he addresses the importance of reverse typing in donor ABO testing and its role in ensuring transfusion safety.

Peter C. Ligthart, MSc, is a seasoned scientist and educator specializing in immunohematology. Since 1989, he has worked at Sanquin’s Red Cell Reference Laboratory in Amsterdam, where he currently serves as both scientist and trainer. His expertise includes blood group serology, molecular typing, and compatibility testing. Peter began his career after completing laboratory technology training in 1988, later earning an MSc in Molecular Life Sciences from HAN University in 2021. At Sanquin, he has led complex serological investigations, such as antibody identification during pregnancy and cases of immune hemolytic anemia, and has contributed to research on blood group antigens and alleles. In addition to his laboratory work, he teaches at HAN Technical University and the Amstel Academy, and has held leadership positions in the Dutch Society of Blood Transfusion. He is also active in the International Society of Blood Transfusion, participating in working parties on immunohematology and blood group terminology.
