Innovation Through Collaboration: Advancing Evidence-Based
Practices in Immunohematology
Innovation in immunohematology is driven not only by scientific discovery but also by collaborative efforts that translate new technologies into evidence-based laboratory practice. This symposium highlights three international collaborative studies addressing important challenges in modern transfusion medicine.
- Presentation 1: The first presentation explores the standardization of automated gel isoagglutinin titration to support safe implementation of low-titer group O whole blood programs.
- Presentation 2: The second examines the comparative performance of different column agglutination technology methods for red blood cell alloantibody identification, providing insights into analytical performance and workflow optimization.
- Presentation 3: The third focuses on overcoming anti-CD38 interference in pre-transfusion testing through evaluation of a novel anti-idiotypic neutralizing reagent that preserves detection of clinically significant alloantibodies.
Together, these studies demonstrate how collaboration between clinical laboratories and industry partners can generate robust evidence that supports laboratory standardization, improves transfusion safety, and facilitates the adoption of innovative diagnostic solutions into routine immunohematology.
Objectives:
- Review the role of ABO antibody titration in low-titer group O whole blood programs and approaches to improving the accuracy and performance of the test
- Examine the validation pathway for a new antibody titration method, including its impact on the identification of high-titer group O blood donors and donor deferral rates.
- Explain complementary approaches for evaluating the efficiency of red blood cell identification panels using scoring criteria that consider resolution efficiency and the number of reagent cells required
- Review current approaches to neutralizing anti-CD38 interference in pre-transfusion testing and discuss emerging solutions, including peer-reviewed results from a multicenter study evaluating a new anti-idiotypic reagent for daratumumab neutralization

Comparative Evaluation of Column Agglutination Technology Methods for Red Blood Cell Alloantibody Identification
Accurate identification and exclusion of red blood cell (RBC) alloantibodies are essential in transfusion medicine and antenatal testing to ensure compatible transfusion and assess the risk of hemolytic disease of the fetus and newborn. The complexity of antibody mixtures encountered in routine immunohematology practice highlights the need to compare antibody identification methodologies. This study compared the analytical and operational performance of RBC antibody identification panels from different column agglutination technology (CAT) manufacturers.
Streamlining the Labor-Intensive Process of Antibody Identifications in a Transfusion Medicine Laboratory Through Antibody Identification Software
Workforce limitations in transfusion medicine and immunohematology have increased reliance on decision-support technologies. These laboratories are staffed by generalists rotating across all laboratory disciplines. Integration of Bio-Rad’s IH-AbID Software with automated serologic testing reduces reliance on manual exclusion/interpretation, expertise, labor demands, and supports timely antibody identifications.
Correlation of Tube and Automated Gel Isoagglutinin Titers for Low-titer Group O Whole Blood Donor Selection
Low-titer group O whole blood (LTOWB) is increasingly used in trauma resuscitation due to its physiological composition and logistical advantages. Control of anti-A and anti-B isoagglutinin titers is critical to minimize hemolytic transfusion reactions. Conventional tube titration remains widely used but is labor-intensive and subject to variability. Automated gel-based methods may improve standardization; however, correlation with tube-based thresholds (1:100) is required to support clinical implementation without compromising donor availability.
